All exhaust canisters are packed with fiberglass type material to give performance and absorb some of the noise. After time this material breaks down and need replacing. Two Brothers offers a repack kit specifically for their exhaust systems. This will not work too well with any other brand.

It depends on the type of exhaust. Usually you'll start to hear the muffler getting louder over time, which is an indication of needing replacement. 2stroke exhausts will need repacking more often than a 4stroke.

Yes. You will have to cut it to size with a pair of scissors.
